About Us

The Maui Long Term Care Partnership is an island-wide coalition of more than 80 people including older adults, consumers, caregivers, six Regional Representatives, service providers, government officials, educators, business development and community action agencies.

Mission Statement

We are dedicated to establishing and sustaining a comprehensive, coordinated home and community-based model of services for all that will foster quality of life and death with dignity.

Goals

1.   Improve access to information about long-term care services (including residential and support services, education, training, and employment) and availability so that people can make informed choices.

2.  Increase awareness and knowledge at all levels of society about aging, long-term care and supportive services.

3.  Provide education and training to long-term care caregivers in both informal home-based and formal professional situations.

4.  Develop, implement, and sustain an organized long-term care infrastructure for Maui Island that matches the community’s needs and preferences.

Structure

The structure of the Partnership consists of a

Governance

The Partnership is governed by a Core Leadership Team which has the responsibility of providing leadership to the collective efforts of the Partnership as it works to fulfill its mission. 

The Team includes at least one representative from each of 20 partnering organizations and provides general oversight and approval of the broad initiatives and direction undertaken by the Partnership.   It meets approximately once a month to review and discuss Partnership activities, new initiatives, and general Partnership organization and governance issues.

Project Director

Rita Barreras is the Project Director for the Maui Long-Term Care Partnership.  She has a Master’s Degree in Urban Affairs from University of Colorado at Denver, and was formerly the Division Director of Aging and Adult Services for the Colorado Department of Human Services.

Teaching and Learning Community

The Maui Long Term Care Partnership is part of a teaching and learning community that includes other partnerships funded by The Robert Wood Johnson Foundation's Community Partnerships for Older Adults program.

The Teaching and Learning Philosophy encourages people and groups to share their creative approaches and success stories, as well as efforts that perhaps didn’t fare so well. 

Community Partnerships For Older Adults (CPOA) holds Teaching and Learning meetings which bring together grantees, national experts and program staff to exchange experiences and learn from one another. The resulting synergy leaves all participants enriched and informed.

CPOA grantee partners are at the forefront of a growing national effort to improve the long term care systems landscape from the community perspective. By learning locally and sharing nationally, communities are developing and implementing sustainable systems improvements.
